Chronic Disease Care
Patient involvement is key in our Chronic Care Management program. By providing assistance in your home, you will achieve a higher level of independence and a better quality of life through education, innovation and care.
Skilled Nursing
Skilled nursing is a service provided by a Registered Nurse or Licensed Practical Nurse. This service includes a coordinated plan of care that is designed by your physician and includes medications that your physician has prescribed. It also includes monitoring vital signs, giving injections, and providing wound care. Other services are provided as needed.
Speech Therapy
After an accident or stroke, speech therapy can help you recover your swallowing and communication skills. Speech-Language Pathologists (SLP) specializes in the evaluation and treatment of swallowing, cognition/thinking skills and communication disorders. Treatment and therapy may help you regain your independence and improve your quality of life.
Occupational Therapy
The use of Occupational therapy is designed to assist you in regaining your independence. The plan starts by teaching you new ways to eat, put on clothes or comb your hair. Often your independence is regained by performing daily activities by yourself. Occupational therapy is sometimes the only form of therapy ordered by your doctor.
Physical Therapy
The primary goal of physical therapy is to help you regain movement and strength. Our physical therapists will work with you in your home to assist you in performing your daily activities and making progress towards your physical goals.
Home Health Aides
Home Health Aides are an important part of your care because they support your nurse by providing assistance with bathing, dressing and other personal care. Home health aid services are usually only covered by Medicare and private insurance policies if you are receiving skilled nursing services.
Social Workers
When recovering from or managing an illness, physical issues are not the only concern. Medical Social Workers can visit your home to assist you in managing the social and emotional concerns that often accompany health issues.
